Best is the onsite physician which costs us nothing.
Worst is the high annual deductible and out of pocket maximum

The high deductible plan is a good option for those who do not see a doctor very often. It allows you to pay a lower premium every month and instead contribute to a Health Savings Account which holds your money and never expires. ADP will also give you over 1000 dollars a year toward your HSA.
