Many providers opted out due to hassles of dealing with the insurance. Company managed to shift more administrative tasks as well as more of the cost onto the employee, so it was often more cost effective to pay for things without using the insurance. Many employees experienced crippling costs for prescriptions as well.

Premiums are very high and deductibles are high as well. Generally not very good plan, but may very well be a problem everyone is facing in today's healthcare wo's. They do offer a health savings account that they do contribute money annually to which is helpful.
