The plan is a preferred provider option (PPO) design, offered to eligible employees scheduled and working 20 or more hours per week. You may also enroll your eligible dependents.

There is no co pay when services are rendered. They bill you directly but they only pay 80 percent. When having a major bill it can get quite expensive. Vision Care could be better.
