It is the minimum required by law. You have to consider the fact that call centers are basically the closest thing to slavery that is still legal in the United States has a lot to do with it. (Example if you make to many trips to the bathroom you will be terminated).

They give you extra money and let you spend it on the insurance you want (from a list of plans), either PPO or HMO. Keep some of the money if you don't spend it on insurance. Options are good and well-priced.
