Exaggerated an innocuous story to complete fabrication and said I learned from it. I really don't like those questions. Think it's good to have failed professionally but I didn't have much experience making it kind of tricky. My advice is to be prepared for anything. You have to be quick on your feet in any interview process. Also make sure you boast about how great the company is. They love hearing that kind of stuff.
