I described a dynamic where as a group we worked together and created a project, however no on really checked if we were doing the right thing or would be effective. We then failed because the project was too limited and we didn't predict the eventual outcomes. I think they were looking for something along the lines of what was wrong with the group dynamic, not what was wrong with the group.
