ABOUT ALTITUDE CARES

Altitude Cares, a CMS Access Program Participant, is an AI-powered medical group that provides post-discharge and between-visit chronic care management. It proactively follows patients from the point of referral through ongoing care management. Clinician teams, supported by Altitude's platform, take continuous ownership of medication reconciliation, lab follow-through, titration, and care coordination from the moment a patient leaves the hospital or is flagged by their outpatient provider.

Referrals come from two channels: at discharge from hospital, ED, or SNF; and from outpatient specialists and PCPs managing patients with poorly controlled chronic conditions. Once referred, Altitude reaches the patient within 24 hours, completes medication reconciliation by Day 2, conducts a telehealth visit by Day 7–14, and maintains continuous management for as long as clinically needed. Referring providers receive structured updates when — and only when — something changes in management.

Altitude operates care management programs across Rhode Island, Virginia, and other markets. Indiana is the next expansion — bringing direct clinical capacity to busy practices whose patients fall through the gap between visits.

ROLE OVERVIEW

The advisor’s job is to develop Altitude Cares’ Indiana referral network by introducing the services and benefits to discharge teams, specialists, and medical groups serving patients with chronic conditions.

WHAT YOU'LL DO

Build a discharge referral network

• Identify and make introductions to care management and discharge planning operators within hospitals.
• Brief clinical and quality leaders on how we drive penalty reduction and reduce lost to follow-up.
• Get Altitude's clinical ops team in front of hospitalist medical directors and discharge nurse managers at healthcare providers across the urban and metro regions of the state.
• Convert introductions into structured discovery conversations that lead to a referral pathway.

Build outpatient referral network

• Introduce Altitude Cares to internal medicine, cardiology, nephrology, hepatology, and endocrinology department heads and group practice administrators at systems and independent practices.
• Activate PCP network — large, employed group practices and IPA/ACO-affiliated primary care — to establish direct referral relationships for poorly controlled chronic patients.
• Facilitate introductions to ACO and IPA medical directors who can operationalize referral workflows.

Build engaged community

• Attend relevant professional association conferences within Indiana to socialize Altitude Cares and build the right referral partners, as well as channel partnership.
• Plan and execute small clinical roundtables and dinners among inpatient and outpatient providers focused on effective use of clinical intelligence and AI and strategies to improve handoffs.
• Coordinate Altitude's presence at Indiana clinical association events.

Surface opportunities and intelligence

• Maintain engagement tracker: who's been contacted, by whom, stage, and what's needed next.
• Identify where Altitude's referral model, consent flow, or reporting needs to be adjusted based on feedback from the Altitude Cares Referral Network.
• Brief the Altitude partnerships team monthly on what's working, what isn't, and where to focus.

WHO WE'RE LOOKING FOR

Required

• Active, current connections with Indiana hospital systems and outpatient specialty practices; strong familiarity with Care Management and Clinical Directors at local health systems and provider groups.
• Based in Indiana
• Clinical, pharma, or health care business development background with a track record in care transitions, discharge planning, post-acute care, or chronic disease management
• Comfort operating independently: you own your own calendar, deliverables, and relationships.
• Strong sales orientation, entrepreneurial

Strongly Preferred

• Background in nursing, social work, health care administration, or similar
• Familiarity with post-discharge care management workflows, clinical best practices, and billing.

ENGAGEMENT TERMS

Contract structure: 1099, billed monthly against agreed scope and hour band

Compensation: Competitive market rate for a senior clinical market development advisor, commensurate with experience

Travel: In-state travel within Indiana reimbursed

Term: 6-month initial term, renewable at mutual option; 30-day written notice to terminate
