Responsibilities

• Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine care needs.
• Monitor and manage remote patient data through RPM technology, ensuring timely intervention when necessary.
• Collaborate with healthcare providers to develop and implement personalized care plans that address chronic condition
• Perform comprehensive medical record documentation adhering to HIPAA regulations while maintaining accuracy in medical terminology and diagnostic evaluation records.
• Provide exceptional patient service through effective communication, education on health management strategies, and support for discharge instructions or outpatient treatment plans.

Requirements

• Proven experience of working in a CCM / RPM program
• Excellent understanding of physiology principles, vital signs assessment, and medical terminology essential for accurate case management.
• Ability to navigate medical records systems while maintaining strict compliance with HIPAA standards.

Pay: $17.00 - $20.00 per hour
