Position Summary

The Initiating Visit Clinician virtual clinical consultations (initiating visits) and care plan oversight. 35+ state licenses required. Applications with fewer will be dispositioned.

You work in a virtual clinic environment with a team of six assigned nurses who intake and prep the visit and complete the care plan at the end of the visit.

This role is consult-based and focused on patient care excellence, CM regulatory compliance, and care plan validation.

The Clinician ensures that the consults and care plans meet Medicare requirements, clinical best practices, and board-level standards while supporting RN Care Managers and Care Professionals in delivering structured, defensible, and audit-ready care.

Core Responsibilities

Clinical Consultations

• Scheduled clinical consults
• Escalation case consultations
• Compliant consult documentation
• Participation in quality or compliance meetings as needed for your completed consults

Medicare Compliance & Regulatory Alignment

• Ensure compliance with CCM (99490, 99487, 99491, etc.) billing requirements.
• Validate medical necessity for enrolled patients.
• Support audit readiness and documentation defensibility.
• Partner with compliance and coding teams to address regulatory risk. (if needed)

Care Plan Integrity & Quality Assurance

• Provide clinical input on wellness-based care plan templates.

Team Collaboration

• Partner with RN Care Managers and Care Professionals to ensure quality of care.
• Provide structured consult guidance for escalated patient cases.
• Support and/or recommend specialist integration into care plans.

Required Qualifications

• MD, DO, PA or NP
• Medicare enrollment and good standing
• Active, unrestricted license in all 35 or more states
• Understanding of CMS documentation and billing requirements
• Experience working in telehealth or remote care environments
• Willingness to complete consults during operating hours 9am-7pm CST.

Preferred Qualifications

• Experience with CCM or value-based care models
• Experience in geriatrics, internal medicine, or family medicine

Rate Structure

• $120-300/hr tiered depending on licensure, availability provided and off hours coverage
• Flexible scheduling: You decide the hour blocks you work
• Minimum half day 4 hour block minimum
• Work as little as one day a week up to five (Closed Weekends)
