Who We Are
We are a Louisville-based healthcare company on a mission to transform post-acute care across Kentucky. We are building the Commonwealth's first statewide, value-based skilled nursing facility (SNF) network — aligning high-performing providers with major Medicare Advantage plans to improve patient outcomes, cut avoidable hospitalizations, and generate shared savings for our members.
Our founding partners include one of Kentucky's largest post-acute providers and a major state university as an equity partner. We are capitalized, in growth mode, and looking for the builder-leader who will take this organization from launch to statewide scale.
The Opportunity
This is an inaugural CEO role — the single most consequential hire in our history. You will have a blank canvas, Board support, proprietary technology (AI-assisted referral coordination, advanced claims analytics), and a founding member base of 20–25 SNF centers ready to grow. Your mandate is to build the network, close the managed care contracts, and deliver profitability by Year 3.

What You'll Do
Strategic & Board Leadership
- Define governance, finalize the Network Charter, and own the long-term strategic roadmap with the Board.
- Serve as the external face of the organization with investors, payers, regulators, and healthcare system partners.
- Lead financial modeling across best/expected/worst-case scenarios and report P&L performance to the Board.
Network Development
- Recruit, credential, and enroll SNF members — starting with 20–25 founding centers and expanding to 90+ statewide by Year 5.
- Implement quality credentialing standards, a value-based scorecard, and contiguous county-level coverage across Kentucky.
Managed Care Contracting
- Lead payer negotiations with Anthem, Humana, and United to secure value-based Medicare Advantage contracts.
- Execute the gain-sharing model and establish the organization as Kentucky's only statewide post-acute MCO contracting entity.
Clinical Quality & Technology
- Oversee deployment of readmission reduction protocols and network-wide clinical KPIs.
- Direct the AI-powered discharge coordination platform and advanced claims analytics to drive member performance and network intelligence.
Team & Financial Leadership
- Hire and develop an initial 7-person management team across six functional areas.
- Manage the investment phase (Years 1–2), drive breakeven by Year 3, and lead investor communications under Regulation D private placement requirements.
What You'll Bring
Required
- 10+ years of progressive healthcare leadership; at least 5 years as a CEO, COO, President, or equivalent.
- Hands-on experience in post-acute care, skilled nursing, or long-term care — as an operator, executive, or network leader.
- Demonstrated success in Medicare Advantage contracting, value-based care, or ACO/ACE leadership.
- Proven ability to build healthcare networks or clinically integrated networks (CINs) from the ground up.
- Experience with investor relations, private equity, or capital raise processes in a healthcare startup or growth-stage environment.
- Master's degree (MHA, MBA, MPH, or clinical advanced degree) strongly preferred.
Preferred
- Existing relationships with Anthem, Humana, or United Medicare Advantage plans in Kentucky.
- Experience with Institutional Special Needs Plans (I-SNPs) or capitated post-acute payment models.
- Familiarity with AI-assisted discharge coordination and healthcare data analytics platforms.
- Established network among Kentucky SNF operators, hospital systems, or state healthcare policy leaders.
