Carnegie East House is New York City’s premier enriched housing residence for older adults who need assistance with their activities of daily living. Reporting to the Executive Director, the Director of Health and Wellness is a senior manager who is responsible for all aspects ofresident care, our medication assistance program, New York State Department of Health compliance and documentation and training and supervising our LPNs on staff and our Social Worker/Case Manager. The Director of Health and Wellness must work in a positive, collaborative manner with residents, family representatives, physicians, and staff members throughout the organization. The Director of Health and Wellness must be a registered nurse who has a strong clinical background, loves working with older adults and has a warm, inviting personality. The Director of Health and Wellness establish all organizational policies and practices that comply with the enriched housing regulations.

• Supervises and works collaboratively with the LPNs on staff.
• Collaborates and coordinates resident care with the various medical professionals who provide care at Carnegie East House.
• Oversees all aspects of the Medication Assistance Program, including hiring, supervising, and training the medication technicians and ensuring all documentation is accurate and up to date. Coordinates adequate supply of medications with various pharmacies and family representatives as appropriate.
• Participates in weekly administrative staff meetings and reviews specific resident care and communicates policy and procedural changes as needed.
• Conducts ongoing in-service training for staff regarding resident health needs and best health practices.
• Assists in the orientation of new personnel, interns, and volunteers.
• Develops resident health promotion and disease prevention programs, including recommending physician appointments when needed, immunizations as appropriate, hydration during periods of excessive heat, etc.
• Responsible for overall compliance, reporting (internal and on the Health Commerce System), quality assurance, and documentation with all New York State Department of Health regulations for Enriched Housing.
• Responsible for establishing a comprehensive Quality Assurance Program that ensures Carnegie East House complies with the enriched housing regulations. Determining which aspects of our operations need to be monitored, the frequency with which they need to be monitored, how we will measure them, document our measurement efforts and follow up on our findings.
• Reviews weekly/monthly/quarterly apartment inspection reports and put policies/procedures in place to comply with DOH regulations.
• Reviews assessments and case notes to ensure that Social Work case notes are in compliance with DOH regulations.
• Reviews all admissions paperwork with Director of Marketing to ensure that all are in compliance with DOH Regulations, including reviewing the Chronological Admissions/Discharge Ledger and Census.
• Reviews all medical evaluations (3122) for completeness and timeliness.
• Reviews Incident Reports for completeness and trends.
• Reviews medication management program to ensure all medication is being administered.
• Conducts Annual Resident Satisfaction Survey and other periodic resident surveys, as needed, and follows up on unfavorable reviews and suggestions.
• Reviews, modifies, and creates comprehensive organizational policies and procedures to ensure that Carnegie East House complies with New York State Department of Health regulations.
• Responsible for all aspects of our Department of Health Evacuation Plan and obtaining and managing the relationships between Carnegie East House and our transferring receiving facilities and transportation agreements. Responsible for overseeing regular evacuation drills.
• Working in conjunction with the Social Worker and nursing staff, oversees the assessment process of all new and current residents to determine if they meet our admissions and retention criteria to live at Carnegie East House. If an existing resident does not meet our retention criteria, works with the resident, resident’s representative, and Case Manager on a suitable placement.
• Responsible for all aspects of the Health Commerce System, including responding to all HERDS surveys, adding and deleting users and distributing new Dear Administrator Letters and other relevant information to relevant staff in a timely basis. Completes daily DOH census and monitors all PPE inventory during pandemic.
• Responsible for initiating and answering all New York State Department of Health correspondence, including our Electronic Plans of Corrections.
• Supervises and works collaboratively with the nurses to ensure compliance with 3122’s, medication management program, self- med management, incident reports, etc.
• Participates in weekly administrative staff meetings and reviews specific resident care issues and communicates policy and procedural changes as needed.
• Conducts ongoing in-service training for staff regarding resident health needs and best health practices. conducts quarterly meetings with each department and their manager to ensure that each department is in compliance with EHP regulations.
• Acts as a liaison with resident family members regarding changes in resident status and any possible interventions to be instituted, such as medication management, hiring a private duty aide, the need for a higher level of care, etc.

Pay: From $130,000.00 per year
