We are seeking a proactive and detail-oriented Transition Coordinator to join our dynamic healthcare team. Your expertise in service operations, clinical workflows, and case management will drive efficient discharge planning, coordination with multidisciplinary teams, and communication with circles of care. Responsibilities

• Coordinate patient discharges from acute care (LTC), Treatment Centers and other qualifying institutions, ensuring timely and safe transitions.
• Collaborate with medical teams, social workers, case managers, and outpatient providers to develop comprehensive discharge plans aligned with patient needs.
• Manage intake processes by assessing patient eligibility for services such as county benefits, SSA benefits, behavioral health programs, or community support services.
• Utilize Microsoft and other company systems to document clinical information, discharge summaries, and utilization review data accurately.
• Conduct utilization management and review to ensure appropriate level of care outcomes, based on care/discharge conferences and service standards.
• Facilitate communication between hospitals, outpatient facilities, insurance providers, and community resources to coordinate patient transitions effectively.
• Ensure compliance with HIPAA regulations by safeguarding patient information during all stages of the transition process.
• Ability to develop Transition Plan Tools and follow through with supporting patient towards meeting their community goals.
• Utilize abilities and supports to seek and discover resolutions and problem-solve in housing and community stability.

Qualifications

• Proven experience in hospital/clinic/direct support settings with a strong background in service and care outcomes and stability.
• Knowledge of community waivers, housing types/options, and community resources and connections.
• Experience working within complex levels of cares of varied diversities and needs.
• Familiarity with MHCP Fee-for-Service systems and discharge planning procedures.
• Strong understanding of social service practices, behavioral health considerations, and managed care protocols.
• Excellent communication skills for engaging with multidisciplinary teams, patients, circles of care, and external providers.

Pay: $22.00 - $24.00 per hour
