The Hospice Nurse Preceptor supports clinical excellence by onboarding, coaching, and developing newly hired hospice clinicians to ensure they are prepared to deliver high-quality, compassionate, and compliant patient care.
This role functions in partnership with the Director of Nursing and clinical leadership to support orientation, competency development, and ongoing staff support. The Hospice Nurse Preceptor does not provide independent clinical supervision or performance management but serves as a clinical mentor and educator.
Core Responsibilities:
· Lead and support the orientation of newly hired hospice clinicians, including education on policies, procedures, documentation standards, EMR workflows, and regulatory requirements
· Facilitate structured onboarding experiences aligned with organizational standards and clinical expectations
· Provide real-time education during patient care encounters
· Serve as a role model by demonstrating best practices in hospice care delivery
· Provide timely, constructive feedback to support clinical growth and competency development
· Support development of clinical judgment, symptom management skills, and patient/family communication
· Conduct patient home visits as part of precepting responsibilities
· Deliver skilled, compassionate hospice nursing care consistent with the plan of care and physician orders
· Reinforce proper documentation practices and regulatory compliance during field training
· Support clinicians in completing accurate, timely, and compliant documentation in the electronic medical record (EMR)
· Reinforce standards in accordance with CMS Conditions of Participation (42 CFR Part 418), state regulations, and organizational policies
· Identify documentation gaps and communicate trends to clinical leadership
· Partner with Director of Nursing, ADON, and clinical leadership to identify learning needs and support performance improvement initiatives
· Provide feedback on onboarding effectiveness and opportunities for education enhancement
· Support Quality Assessment and Performance Improvement (QAPI) activities as assigned
Basic Qualifications/Requirements:
· Current, unencumbered Registered Nurse (RN) license in the state of residence
· Minimum of two (2) years of clinical nursing experience
· Minimum of one (1) year of hospice experience preferred
· Strong communication, coaching, and interpersonal skills
· Demonstrated ability to educate and mentor clinical staff
· Previous preceptor, mentor, or clinical educator experience preferred
· CHPN and/or NPD-BC certification preferred
· Valid driver’s license, reliable transportation, and valid auto insurance
· Applicants must be legally authorized to work in the United States
Physical Requirements:
· Ability to lift, move, or assist patients up to 50 pounds using proper body mechanics or assistive devices
· Ability to stand and walk for extended periods during patient visits
· Ability to bend, kneel, and stoop as needed for patient care tasks
· Sufficient visual and auditory ability to monitor patients and ensure safety
· Manual dexterity to operate medical equipment and complete documentation.
Work Environment:
· Patient homes and community settings with varying conditions
· Office and clinical environments as needed
· Regular travel between patient locations
· Interaction with patients and families in emotionally sensitive situations
· Collaboration with interdisciplinary group (IDG) members and leadership
· May require occasional evenings or weekends based on orientation needs
Pay: $90,000.00 - $95,000.00 per year
