There is a lot of the blame game going on. Any time anything doesn't flow at a normal pace, people are put into the fire. This happens most on a contract work, no so much if you're charging overhead. Also, there is a very standard career progression. HR/management give promotions 90% based on years worked rather than actual skill. There are plenty of times when more qualified individuals are overlooked because they haven't put their time in.
