Starting with 2014 plans, vision was broken out as a $1 per month premium. Coverage is through VSP which really limits the "in-network" provider selection. Go out of network and get $50 for an exam (which is going to be less than half of your charge). Contact lens reimbursement lags behind other vision plans at other companies.

I think vision comes with all health plans. I still have to pay money though for my lense cuz my eyes are really terrible.
