There is a $15,000 lifetime maximum total benefit across Cigna and UHC plans for infertility benefits; a separate lifetime maximum of $10,000 applies to drugs to treat infertility. Infertility treatment/testing covers determining the underlying cause, as well as in vitro fertilization, artificial insemination, GIFT or ZIFT.
