Premium plan is not available for purchase. Highest level you can buy is not PPO meaning you will pay $1500 for doctor visits until that deductible is met. So technically this does not qualify for HSA/high deductible yet it is the highest cost benefit plan and listed as the top level. This is inadequate as a healthcare benefit.
