I only use the mutual funds, which are named by the decade in which you hope to retire. Performance has been very satisfactory and the website makes it easy to switch things around and get help/questions answered live on the phone without trouble.

company matches 125% for first 6% contribution. but if you leave the company within 2 years, you lose entire company contribution

The matching is nice. You can contribute a large amount which is pretax. I don't remember the percent they match, but I remember being satisfied with it.

Humana invested a decent percentage into 401k plans. I was very satisfied with my input and the company matching it. You can not touch any of the funds until leaving the company.
