The options have changed since the affordable care act has been introduced. They have a lot of options, to where it is a little over whelming, but they have PPO and HMO options. They vary by state.

It used to be way better. It may not be Ingram and it may be a result of the Affordable Care Act, but I was in a Gold Plan before and I had to downgrade 2 plans (to a Bronze Plan) to keep the same contribution rates. My Gold plan went up several hundred dollars per month.
