Both via major, national provider, but via a local branch in another state, which makes communication and in- vs. out-of-network challenges very annoying and tough to deal with.

The health care plan was very expensive and did not offer as much as other plans through other companies. In addition, if you did not take the health care plan you paid a penalty. Many places will offer to pay you if you don't take the health care plan because it costs them less.
