Pretty much won't cover anything unless you are getting the top plan. Even then you will have issues when trying to find people in certain specialties. Getting them to help you find a mental health physician online is near impossible. Calling them also does not work, you will never get their "follow up email".

It's with United Health Care - which is horrible in the state of Utah. Also, the vision is with eyemed, and dental is through some random company that is also terrible and I've had to switch dentists because my preferred dentist isn't covered.
