Full coverage, yes, but ironically a self-funded plan that got worse and yet more expensive over time . Getting insurance to *actually pay* for a pre-approved surgery after we'd had coverage in force for 2.5 years actually took nearly 2 years and included too many collection letters to keep track of. Incredible.
