United Health Care high deductible plan. The plan is a la carte meaning "choice" and you can go directly to specialists. You pay for everything out of pocket up to the deductible which you can choose $3-4000. Then coinsurance 10-20% until the out of pocket max near $7,000 for a family. The company contributes to an HSA on your behalf and you can contribute up to federal limits using pre-tax money. The money is never taxed if used for medical expenses. I actually like the plan ok, the premiums are not too bad, but they doubled last year, and I expect them to keep rising.

Only HSA or Kaiser, but the by in for HSA is pretty low compared to other employers. No longer offering PPO.
