I was recruited through a recruiter after submitting an application online. I had a call with the recruiter and went through my resume with her. Very basic resume screening. Then she recommended to the hiring manager to be put through a formal interview. I was then scheduled for a phone interview and received my job offer the same day.

Short and concise. Interviewers were not prepared. They were not aware of the interview at all.
