Seniorlink is excited to announced a partnership with an independent practice association in Rhode Island, the Rhode Island Primary Care Physicians Corp (RIPCPC), to pilot Vela, our Care Collaboration Platform. In this pilot, RIPCPC will use Vela to connect its nurse case managers, care coordinators, patient advocates and pharmacists directly with 500 patients and their family caregivers.
This week Seniorlink announced VOICE Dementia Care, our training program designed to engage with and support family caregivers of patients with dementia. The announcement comes after a poster presentation that the CCE team debuted at the International Association of Gerontology and Geriatrics World Congress last week, covering the results from the research study that led to VOICE DC. Congrats to all who worked toward this great accomplishment!
