Do you understand social determinants of health? It quite literally means that your health is mostly dependent upon your socioeconomic situation. Join us in this groundbreaking work for health equity. Solving population health challenges starts on a smaller scale in our communities, schools, and environment. Digital health can expand access to specialty care, increase personalization, improve health literacy, and build a patient-centered data infrastructure and research model. Technology alone is not the answer, but technology built with therapeutic intent, by caring clinicians, has the ability to help us improve individual outcomes — at scale.
