Get to chose between two big companies, BCBS or UnitedHealthCare. The deductible used to be much lower, so over the last couple years it has increased.

Very inexpensive. Family of 3 is about $140 bi-weekly for Medical, Dental and Vision. It usually pays about 85% of charges, additionally covering yearly well-visits (at least most of that, in my experience).
