Best: comprehensive health coverage, low co pays.
Worst: no coverage for LASIK, mandatory annual health questionnaire required to get monthly "rebate" on premiums; questionnaire asks very invasive questions. Caution should be taken when answering these questions as no one is sure who's hands this information gets into.
