Not too expensive but not the best insurance. You will have to pay a good amount out of pocket. They do give the opportunity for earning dollars if you don't smoke and exercise.

No HMOs. High deductibles. There are an in plan and out of plan rates. Overall cost or value is pretty good and there are some health incentive programs. They did help my wife quit smoking.
