Cheated me of $2500 by applying the claim towards deductible instead of preventive care as mentioned in Summary of benefit coverage. Appealed the claim but no response. It was a shocker that they would cheat their own employee. Optum-Rx PBM will deny coverage to save $2 on an ointment but if they are making more money on your head they voluntarily will not disclose cheaper generic options.
