Emergency room visits are out-of-pocket until the deductible is met, after which benefits are paid at 80% until the out-of-pocket maximum is reached. This used to be a set copay amount ($300 or so) if the visit was deemed a true emergency.

Typical corporate plan, reasonable monthly fees for a typical ~$1.5k-2k-ish deductible plan. Bi-annual company contribution to HSA for plan with HSA
Plan with FSA available, no company contributions to that.
