Aledade supports primary care organizations so they can succeed in value-based care. We are the country's largest network of independent primary care, serving more than 2,400 practices and health centers across 46 states and DC, covering nearly 3 million patients.
We don't own or operate practices and clinics - we partner with them. And we give them the data and support they need to keep their patients healthy and succeed financially in value-based care. Through our local and nationwide accountable care organizations (ACOs), we provide preventive guidance so clinicians are rewarded for keeping people healthy, not just for the quantity of procedures they perform. Our health plan contracts enable clinicians to deliver value-based care across traditional Medicare, Medicare Advantage, Medicaid and commercial insurance - meaning patients get high-quality care across insurance programs.
This kind of care saves the health care system money, and we split those savings 50-50 with our partners. To date, Aledade ACOs have generated more than $2 billion in savings, enabling our partners to deliver better care, reward and hire staff, enhance their facilities and stay independent.
Aledade was founded in 2014, and now employs more than 1,400 talented professionals committed to creating a health care system that is good for patients, practices and society.
