Employer pays about 82% and offers an HSA; I don't like that is is a Minnesota insurance company that few outside of the region have heard of because it creates a lot of billing issues with my doctors.

many options to control cost plus you have options to ensure you are covered with PPOs and self directed plans. A con of the plan is that our flex spending accounts seem to be dwindling in size. This may be a government thing versus a company option to be fair.
