Years ago, it was 100% coverage with no premiums, no copays, nothing out of pocket, ever.
These days, it's a high deductible plan, but Microsoft makes a sizeable HSA contribution to offset the deductible. If you're young and healthy, you can actually make money by spending less than the HSA contribution, but if you have a sizeable expense or a chronic condition, you can end up owing a couple grand out of pocket.
Still, you pay no premiums, and the coverage is excellent. More than anything, it's a hassle compared to the no-questions-asked plan from the old days.

fairly good coverage though the deductible has gotten to be much higher than previously
