PPO. Choose provider you want and/or need to see. No referrals necessary. High deductible BUT the FSA card covers that expense for you.

I go to the doctor and don't have to worry about paying anything. Most everything is covered and if it isn't, you get a card that pays for part of your already low deductible

Very affordable, and once you become a member you are treated like family
